Apologies for the lateness of my reply.
This solution was developed for an eZ Publish 3.8 site (with 3.9 compatibility in mind) several years ago. The customer was very happy and has yet to request an update or upgrade.
You are correct in your assumption that the eZ Publish 4.0 represents a major update which almost certainly require changes to this extension in order to properly perform the expected / required correct function of this solution.
I am curious if others have an interest in an eZ Publish 4.0/4.1 compatible BC Simple Subscription?

Hello Peter Scott,
Apologies for the lateness of my reply.
Today I had the pleasure of adding support for eZ Publish 4.x
in the subversion repository of this project, BC Simple Subscription.
This solution was updated for an eZ Publish 4.2 site. The customer was very happy to be helping others with this update or upgrade.
We welcome the community's feedback about using bcsimplesubscription extension with eZ Publish 4.
